  • Overview

    The Social Worker II will provide in home counseling services to children age 17 and under and their families who are living in Mercer County.  Services take place in foster, birth family, and kin placements.  Services should be trauma informed and/or trauma specific depending on the reason for referral. 

    Full Time - 35 Hours Per Week

     

    Work Location:  Home Based supporting the Mercer County area. Valid Driver's License and reliable transportation required as travel through Mercer County required.

     

     

    Schedule: Flexible and varied work schedule, including evenings and weekends required.  Schedule will be determined and coordinated by the Program Administrator based on the needs of the clients.  

     

     

    Responsibilities

    • Work directly with the Program Administrator and meet regularly to review all cases.
    • Maintain caseload as assigned according to the Level of Service in DCPP contract. 
    • Provide education services to parents in Mercer County who are experiencing adjustment difficulties or need increased understanding of the challenges associated with parenting a child who has experienced trauma.
    • Attend monthly case conferences with DCPP, scheduled Family Team Meetings, and unit meetings at CHS. 
    • Attend scheduled trainings.
    • Maintain client documents as required in CHS Clinical Policy Manual.

     

    Qualifications

     

     

    Education Requirements:

    The Social Worker II shall have a Master’s degree from a CSWE-accredited school of social work or CACREP-accredited program of counseling psychology and have the credential of Licensed Social Worker, Licensed Associate Counselor, Licensed Professional Counselor or Licensed Clinical Social Worker.

     

    Work Experience Requirement:

    Must have a minimum of three to five years experience working in an accredited social service agency, working with children and adolescents or in providing social services to children and families; graduate level internships may be considered as part of this experience.
